Affidavit to confirm attendance?

I am in a bind. Please help! I was entrusted with attendance records for this one person who I drove to AA meetings, but somehow I lost the records! I have no idea what I did with them. I had them at one point, then when I went to get them, they are no where to be found. What can I say in an affidavit that this person attended the meetings? Please no lectures ... I have already heard a lot!
